Sierra Vista Fall Sports Permission Forms

Hello Sierra Vista Families!
Welcome to the new school year.  If your child plans to participate in after school sports for the FALL season (Football, Cross Country, Girls Volleyball) please complete the following Sports Permission Packet. 5th and 6th grade students are eligible for Football, Girls Volleyball, and Cross Country.  4th grade students are eligible for Cross Country only in the fall season.  Students MUST have ALL FORMS COMPLETED in order to participate.  Fall Sports Begin Tuesday, August 18th.  These forms are not available in print.  If you have any questions, please call the Sierra Vista Main Office at 559-327-7900 or visit our web page for more information about athletics.
