Winter Sports Permission Slip (Boys/Girls Basketball grades 5-6, Wrestling grades 4-6)

Hello Sierra Vista Families,

If your child plans to participate in after school sports for the Winter season (Basketball or wrestling) please complete the following sports permission packet.  Grades 4-6 are eligible for Wrestling.  Grades 5-6 are eligible for basketball.  Students MUST have ALL FORMS COMPLETED in order to practice and participate.  Winter sports begins Monday November 2nd.  These forms are not available to print.  If you have any questions, please call the Sierra Vista Main Office at 559-327-7900 or visit our web page for more information.
